Optimum Label To Return Equipment
Disconnect the equipment being returned and put it in a box along with any remotes or power cords provided by optimum. Sign in and go to my orders. Print out the return label that was emailed to you during cancellation. Firmly attach the label to the box containing the equipment. Drop it off at one of the authorized drop off locations near you like ups stores, authorized dealers etc.
